### Overview:
We are seeking a dedicated and compassionate Community Support Worker to join our team. ### Key Responsibilities:
- Provide one-on-one support to clients, assisting them in daily activities, social engagement, and personal development.
- Develop and implement individualized support plans based on client assessments and needs.
- Facilitate access to community resources, including healthcare, educational services, and recreational activities.
- Maintain accurate documentation of client progress, interactions, and any incidents in compliance with organizational policies.
- Advocate for clients’ rights and needs within the community, ensuring they receive appropriate services and support.
- Collaborate with multidisciplinary teams, including social workers, healthcare providers, and families, to create a holistic support network.
- Conduct regular follow-ups and assessments to evaluate the effectiveness of support plans and make necessary adjustments.
- Organize and lead community workshops and outreach programs to promote awareness and engagement.
### Required Qualifications:
- Education: GED or High School Diploma but Bachelor’s degree in Social Work, Psychology, or related field.
- Experience: Minimum of 2 years of experience in community support, social services, or a related field.
- Technical Skills: Proficient in Microsoft Office Suite and basic data management systems; familiarity with case management software is a plus.
- Soft Skills:
- Excellent interpersonal and communication skills.
- Strong problem-solving abilities and adaptability.
- Empathetic and patient demeanor with a genuine passion for helping others.
- Ability to work both independently and as part of a team.
### Preferred Qualifications:
- Additional certifications such as Certified Community Support Professional (CCSP) or Mental Health First Aid.
- Experience working with diverse populations, particularly those with mental health or developmental challenges.
- Bilingual abilities in English and another language (Spanish, Amharic, etc.) are highly valued.
