Key Responsibilities:


  • Develop and implement individualized rehabilitation plans tailored to clients' unique needs and goals.
  • Conduct thorough assessments to evaluate clients’ mental health, functional abilities, and support needs.
  • Collaborate with interdisciplinary teams, including therapists, social workers, and medical professionals, to coordinate comprehensive care.
  • Facilitate group and individual therapy sessions that promote recovery and enhance coping strategies.
  • Monitor and document clients' progress, adjusting rehabilitation plans as necessary to ensure optimal outcomes.
  • Provide education and resources to clients and their families regarding mental health and recovery processes.
  • Advocate for clients' needs within the community and assist them in accessing appropriate services and resources.
  • Stay updated on best practices in rehabilitation and mental health services through continuous education and training.

Required Qualifications:


  • Education: Bachelor’s degree in Psychology, Social Work, Rehabilitation Counseling, or a related field; Master’s degree preferred.
  • Experience: A minimum of 2 years of experience in a rehabilitation or mental health setting.
  • Technical Skills:
  • Proficiency in assessment tools and rehabilitation methodologies.
  • Familiarity with Electronic Health Records (EHR) systems.
  • Soft Skills:
  • Excellent communication and interpersonal skills.
  • Strong problem-solving abilities and critical thinking.
  • Empathy and a client-centered approach to care.

Preferred Qualifications:


  • CPRP certification or equivalent credential.
  • Additional certifications in mental health, trauma-informed care, or specialized rehabilitation techniques (e.g., WRAP, ACT).
